Waiver & Release — Free Tryout

I understand that soccer training involves physical activity and possible risk of injury. I confirm that my child is physically able to participate in the free tryout session(s). I release Eagle Stars Soccer Academy, its coaches, staff, and facility partners from liability for injuries or accidents that may occur during training, except in cases of gross negligence. I give permission for emergency medical care if needed and understand that I am responsible for any medical expenses. I also give permission for my child to be photographed or recorded for academy promotional use unless I notify the academy in writing.

I understand that 1 session tryout is a no-obligation trial and that no payment is collected at this stage. After the tryout, the academy may offer a paid enrollment which is entirely my (the client) choice to accept. I have read and agree to the waiver and release.
